The transfer is subject to the 30-year-old completing a medical.
A brief statement on Bayern's website reads: "Mats Hummels is set to return to Borussia Dortmund. FC Bayern and BVB agreed to a deal for the transfer of the center-back for an undisclosed fee on Wednesday."
Although neither side has revealed the details of the transfer, the Guardian reports that Dortmund completed the move after Bayern accepted a bid worth €38 million including add-ons.
Hummels returns to the Westfalenstadion three years after Bayern procured his services from Dortmund for €35 million. He went on to win the Bundesliga title three times with the Bavarians, as well as the DFB-Pokal in May.
He helped Der BVB capture a pair of league titles in 2011 and 2012 and led the club to cup success in 2012.
