The Blues defeated Liverpool 1-0 in the second leg of their semi-final contest at Stamford Bridge on Tuesday, winning 2-1 on aggregate after the first leg ended in a draw at Anfield.
The match needed extra time to be decided, with a classic, bullet header from Branislav Ivanovic in the 94th minute making the difference.
Jose Mourinho and company will now play the winner of the other semi-final tie between Tottenham and Sheffield United, who are set to play their second-leg match tomorrow.
