Cleveland Indians outfielder Josh Naylor collided with teammate Ernie Clement during Sunday's game against the Minnesota Twins and was carted off with an apparent leg injury.

Naylor and Clement ran into each other while attempting to field a blooper from the Twins' Jorge Polanco in the fourth inning. The ball fell for what was ruled a hit as Naylor was sent flying, and he was in visible agony after landing awkwardly on his leg.

"We know that there's a fracture," Indians manager Terry Francona said following the game, according to Zack Meisel of The Athletic. "Other than that, he's being evaluated over at the hospital right now. We'll know more, I hope, in a couple hours. We're not even sure if he can travel home with us."

After trainers tended to him, the cart was summoned to help Naylor off the field. Bradley Zimmer entered the game as a defensive replacement.

Naylor, 24, is hitting .253/.301/.399 with seven homers this year through 69 games.
