Bosa stayed on the ground after the play and his teammates gathered around him before the cart arrived on the field.
The 49ers entered Week 2 already without Dee Ford - who's battling a neck injury - and star tight end George Kittle.
Thomas amassed two tackles before exiting Sunday.
